Requirements:
- Letter from your doctor stating that the handler of the service dog is in need or can benefit from a service dog.
- List of medical, mental, physical ailments/disabilities, and task you need your service dog to perform
- If you need us to provide or recommend a dog for your specific needs please let us know.
- All personal dogs will be evaluated before being enrolled into a program. The evaluation will test: behavioral issues, nerves, stability, temperment, and work ability.
